Use the linked US dog/cat certificate after examination by a USDA-accredited veterinarian and APHIS endorsement. It is valid thirty days after issue. Carry the ink-signed, embossed endorsed original.

Rabies vaccination must be current at export; record the vaccine, batch, date and end of validity. Examination must show no infectious/contagious disease or known dangerous exposure, including rabies. The pet must originate outside a rabies-quarantined premises and have no known rabies exposure. The model sets no numeric vaccination waiting period.
