Use the linked US dog/cat certificate after examination by a USDA-accredited veterinarian and APHIS endorsement. It is valid thirty days after issue. Digital endorsement is accepted; carry the printed endorsed certificate. The actual downloaded model is marked MAY 2026, although the link label says June 2026.

The model records a microchip number and the veterinarian certifies verification of each number entered in box 21. Confirm the identification accepted for the actual pet; this verification sentence alone is not treated here as an independently established universal implantation mandate.

Rabies vaccination must be current at export; record the vaccine, batch, date and end of validity. Examination must show no infectious/contagious disease or known dangerous exposure, including rabies. The pet must originate outside a rabies-quarantined premises and have no known rabies exposure. The model sets no numeric vaccination waiting period.
