A USDA-accredited veterinarian issues the linked certificate after examination, then APHIS endorses it. It is valid 30 days from issue. Digital endorsement is accepted and the printed endorsed certificate accompanies the pet.

The US model requires US residence since birth or for the preceding six months; no rabies at the origin premises in the past 12 months; and examination showing no clinical rabies or contagious disease. It permits microchip number or name; this is not proof of compulsory chip implantation.
